In the past seven years, the Swahilipot Hub Foundation (SPH) has been dedicated to supporting youth in overcoming challenges and thriving. Through strategic partnerships with communities, government entities, CSOs, and local leaders, the foundation leverages the strengths and diversity of individuals from all backgrounds to empower young people and enhance their quality of life. Currently, the Swahilipot Hub Foundation is actively searching for passionate and motivated individuals to join as case managers for the Case Management project working under the project Manager’s leadership. The case managers will play a crucial role in facilitating an onground structured Mentorship program to opportunity youth to enable them access economic opportunities. The case manager will work with the Youth Hub Networks at the grassroot level and program team including the SPH team at large in contributing to the overall mission of Swahilipot Hub Foundation ‘to enable the community to develop themselves.’

Goal:

The overarching goal of the Case Management Project is to develop and implement a comprehensive case management approach to connect 60,000 youth across Mombasa County and its six sub-counties to diverse opportunities and sustainable employment. By focusing on case management, SPH seeks to provide tailored support and guidance to empower young individuals, enabling them to access and thrive in various educational, vocational, and entrepreneurial pathways. As the case manager, your crucial role in accomplishing the goal of this initiative involves facilitating a structured mentorship program for opportunity youth. This includes closely monitoring and evaluating the individual journeys of each case, as well as referring them to other necessary services through the Youth Hub Networks and other partners within the ecosystem.

Key responsibilities:

The case manager will play a vital role in co-designing the mentorship program, mobilizing opportunity youth (OY), and facilitating structured mentorship to assist them in accomplishing their personal and professional objectives. The case manager will also Monitor and evaluate the progress of OY on a case-by-case basis, identifying any challenges or additional needs.

Additionally, the case manager must stay updated on emerging needs and opportunities for OY and actively contribute to the continuous co-designing of the program alongside the project team. Building a strong relationship with the Youth Hub Networks and gaining familiarity with their programs and services is also essential to act as a reliable referral point for additional services required by OY. Your exceptional interpersonal skills, knowledge of youth development principles, and ability to establish positive relationships will significantly contribute to the success of our mentorship program.

Responsibilities

  1. Facilitate structured mentorship sessions for opportunity youth (OY) in accordance with the program guidelines and objectives.

  2. Build strong rapport and provide guidance, support, and encouragement to opportunity youth (cases), helping them set and achieve personal and professional goals.

  3. Monitor and evaluate the progress of OY on a case-by-case basis, identifying any challenges or additional needs.

  4. Collaborate with the Case Management team to refer mentees to other necessary services available through the Youth Hub Networks and other ecosystem partners.

  5. Maintain accurate records and documentation of mentorship sessions, progress, and outcomes.

  6. Attend regular meetings and training sessions to enhance mentoring skills and stay updated on

    program developments.

  7. Collaborate with the Project management team and other case managers to ensure the effective

    implementation of the mentorship program.

  8. Communicate regularly with the Project Manager, providing updates on mentee progress and any

    issues or concerns that may arise.

  9. Stay updated on relevant policies, regulations, and funding opportunities related to youth

    programming and make recommendations for program enhancements.

  10. Any other duties as may be assigned by the Project Manager

QUALIFICATIONS AND EXPERIENCE

  1. Diploma in any relevant field (e.g., social work, psychology, education, e.t.c) or equivalent practical experience.

  2. Proven experience in mentoring or coaching youth, preferably within a community or non-profit setting.

  3. Strong interpersonal and communication skills, with the ability to establish trust and build positive relationships with PY.

  4. Knowledge of youth development principles and experience in guiding and supporting OY in their personal and professional growth.

  5. Cultural sensitivity and awareness, with the ability to work effectively with diverse groups of youth from various backgrounds.

  6. Excellent organizational skills and the ability to manage multiple tasks and priorities.

  7. Proficiency in using computer applications for record keeping and documentation; MS Suite.

8. Familiarity with the local context of Mombasa County and understanding of the challenges and opportunities faced by youth in the region.

Note: This position will work closely with the Project Manager, who will provide guidance, support, and supervision. The Mentor will report directly to the Project Manager and contribute to the successful implementation of the youth programming initiatives in Mombasa County.
